How Coda supports physician-directed care.

Coda is a physician-directed system for continuous, personalized care. It is designed to help licensed healthcare professionals maintain longitudinal patient context, recognize moments that may merit review, and carry bounded clinical work between encounters.

Coda's current intended use is decision support for non-emergency, non-time-critical care. It is not an autonomous clinician and does not replace examination, professional judgment, or the physician-patient relationship.

With authorized connections, Coda can observe relevant records, results, messages, schedules, and selected home data. A source change, approaching encounter, due follow-up, or physician direction can give Coda a reason to review a patient's care.

Coda is designed to reconstruct the prior decision, current state, and material uncertainty; fill bounded information gaps through approved sources; and, when useful, give the physician concise evidence-linked judgment and options. It can carry an explicit follow-up forward for later reassessment while keeping completed work available for inspection.

Coda works only with patient information made available through authorized sources or provided by the physician. It is designed to preserve which patient facts were used, where and when they came from, and whether relevant information is current, incomplete, unavailable, or conflicting.

Patient-specific claims should remain traceable to the exact source material behind them. General medical knowledge may inform clinical reasoning, but it is not a substitute for patient evidence, and material evidence or knowledge limits should be visible to the reviewing professional.

Before making a decision, the physician should be able to inspect Coda's assessment or recommendation, the patient-specific facts and sources that support it, the rationale presented, material uncertainty, reasonable alternatives and tradeoffs, relevant limitations, and any question that remains unresolved.

The physician may accept, change, reject, or ask Coda to reconsider an output. New information and physician corrections remain part of the continuing patient context rather than being treated as approval of Coda's earlier interpretation.

Coda may recommend what it would do and explain why, but it does not determine care. The licensed healthcare professional makes final decisions about diagnosis, treatment, medications, orders, and communication and approves any action that follows.

The current Coda clinical product does not independently send a patient message, place an order, change a medication, write back to a clinical record, or perform another clinical action. Any later action capability requires explicit, action-specific authority, workflow controls, and a governing agreement.

Coda is not intended for emergencies, autonomous monitoring, alarms, or time-critical directives. The current product does not analyze raw medical images, waveforms, continuous physiological signal streams, or raw diagnostic signals; any future function that does would require its own product, clinical, and regulatory review.

Clinical performance is specific to the function, intended users, patient population, information available, workflow, and setting. Before a clinical deployment, Coda Health and the participating healthcare organization must define and validate those conditions, along with known limitations, escalation paths, monitoring, security, and operational controls. Deployment-specific labeling and agreements control if they differ from this general description.

Duet is Coda Health's separate, patient-directed iPhone app. It helps people organize health information they choose to connect, review supported observations, and prepare information to share in a conversation with a healthcare professional.
